Overview
Build a post-visit survey as a registration form with the survey setting turned on, then optionally schedule it to send automatically after appointments. Creating the survey form doesn't schedule it on its own — the form is the survey content, and the Communication Timeline controls when it's delivered.
Before you begin
- Access to Config > Registration (and Config > Communication Timeline if you want it to send automatically).
Create the survey
- Go to Config > Registration.
- In the Forms tab, click Add New Registration Form.
- In the Form Details tab, complete the basic setup — Title, Description, Sequence, and How often is this form required?
- Open the Miscellaneous tab.
- Turn on Is this a post-visit survey? This tells the system the form should behave as a survey instead of a standard registration form.
- Complete any other optional settings on that tab if needed, such as age requirements, tasklist settings, or public form settings.
- Open the Sections tab.
- Add the sections you want included, using the Add a section dropdown to add existing sections.
- Arrange the sections in the order you want them to appear.
- Click Save.
Customize the survey
- After saving, use Actions > Open in Designer to make more detailed changes to the survey layout or fields.
- Use Preview or Preview & Test to review the patient experience.
Make the survey send automatically
To have the survey go out automatically after appointments:
- Go to Configuration > Reminders > Communication Timeline.
- Click the day you want the survey to send after the visit.
- Set Event Type to Survey.
- Choose the Communication Method.
- Set the timing and time of day.
- Add any needed assignments.
- Click Save.
Troubleshooting
- If you created the survey but it never sends, then remember the form is only the content. You still need a Survey event on the Communication Timeline to deliver it.
- If the form behaves like a standard registration form, then confirm Is this a post-visit survey? is turned on in the Miscellaneous tab.
