Overview
The Surveys dashboard helps your team track how many surveys go out, how many are completed, and how patients respond to each survey question over time — broken down by provider, location, and resource. Use it to monitor completion rate and watch patient feedback trends.
Before you begin
- Access to the Home screen dashboard view selector.
Steps
- Go to Home, open the dashboard view selector in the top right, and choose Surveys.
- Set your date range using the From and To filters at the top. The dashboard updates based on that range.
- Review the top summary cards: Surveys Completed, Surveys Sent, and Completion Rate (the percentage of sent surveys completed). This is the fastest read on overall performance.
- Review the Provider Performance section to compare survey results by provider and spot stronger or weaker response results.
- Review the question trend graphs. Each survey question has a line graph showing the question title, the response trend over time, and weekly changes in average responses. Use these to see whether scores are improving, whether a recent workflow change affected feedback, and which topics are trending up or down.
- Review results by location and resource. For each question, grouped tables (Location | [Question] and Resource | [Question]) show the average response grouped by location or resource/provider — useful for comparing sites and providers and finding where scores are strongest or weakest.
- Export the data if needed: in the Reports panel, click Survey CSV Extract to open a CSV export for the selected date range, for spreadsheet analysis or reporting outside the dashboard.
Best ways to use it
Track whether survey completion is improving, compare performance across providers, compare patient feedback across locations, watch specific questions over time, and export results for deeper review or leadership reporting.
Troubleshooting
- If the numbers don't answer your question, then check your date range — the dashboard is only as useful as the timeframe you choose, so make sure it matches what you're investigating.
- If you're reviewing a recent change, then compare before-and-after date ranges to see whether responses shifted.
